APPLICATION PROCEDURES RULES:

A. Any FCSTAT member or group of members may nominate a person in each category.
B. Recipients of the state awards will be selected from the regional winners.
C. Materials not submitted in the format requested will not be considered.
D. The number after each item is the total number of points that may be awarded in that category.
E. This form may be duplicated.
F. Region Teacher and Rookie Teacher applicants are requested to submit a color 5x7 photograph.
G. It is suggested that materials be submitted by someone other than the nominee.
H. Five years must elapse between nominations for winners of the region teacher award.
I. FCSTAT Board Members are ineligible.

EACH NOMINATION SHOULD INCLUDE THE FOLLOWING INFORMATION:

I. Biographical Information for all Categories not to exceed two pages, typed and single spaced.

A. Name, title, address, telephone numbers (office and home) and school districts
where applicable 5
B. Educational background 5
C. Employment background 5
D. Professional organizations, honors and recognition 5
E. Other (family, etc.) 0
Total 20

EACH NOMINATION SHOULD INCLUDE SUPPORT MATERIAL FOR THE CATEGORY NOMINATED

II. Support Material
A. Categories

1. Region/State Teacher and Rookie Teacher of the Year
Address “a.” through “d.” in no more than two pages, typed and single spaced.
Title paragraphs to correspond with each criteria.

a. Impact of effective and/or exemplary family and consumer sciences programs 35
b. Innovative/effective teaching methods and techniques 30
c. Involvement in FCCLA 20
d. Involvement in FCSTAT 15
e. Letters of recommendations (optional) 0

Three letters of recommendation, and no more than three, may be included.
Letters may be written by co-workers, administrators, students, parents, advisory
board members, etc.
Total 100

2. Administrators, Career and Technology Administrators, Career and Technology Supervisors,
Counselors, Golden Service and Distinguished Service
Address “a.” through “d.” in no more than two pages, typed and single spaced.
Title paragraphs to correspond with each criteria in the order listed below..

a. Demonstrated significant support of family and consumer sciences programs and
family and consumer sciences teachers 30
b. Contributed to the promotion of family and consumer sciences programs 25
c. Demonstrated support of FCCLA activities 25
d. Demonstrated positive role model 20
